- If you are a Michigan Licensed Manufacturer/Wholesaler Distributor you are required to identify your Pharmacist-in-Charge (PIC). To apply, you may download the following application: Miscellaneous Pharmacy Change Application
- If you are an Out-of-State Licensed Manufacturer/Wholesaler Distributor you are required to identify your Pharmacist-in-Charge (PIC). The PIC may be an out-of-state licensed Pharmacist.
- If you are an Out-of-State Pharmacy your Pharmacist-in-Charge is required to be licensed in Michigan. The Out-of-State Pharmacy is required to identify its Pharmacist-in-Charge (Michigan Pharmacist license number required). To apply, you may download the following application: Pharmacist Endorsement Application.
- If you are a Compounding Pharmacy as defined in the Public Act 280 of 2014, you are required to be licensed in Michigan. To apply, you may download the following application: Pharmacy License Application.
