On May 12, 2020 Governor Kevin Stitt enacted an Executive Order that removed all restrictions regarding prescriptions for Hydroxychloroquine and Chloroquine.  

An OSBP COVID-19 waiver is no longer necessary for Hydroxychloroquine/Chloroquine prescriptions. 

The COVID-19 waiver request form is still available for a request for consideration to waive Oklahoma Pharmacy Rules during the COVID-19 pandemic.
