At the June 29-30, 2016, meeting of the Iowa Board of Pharmacy (Board), the following public actions were taken:
Anthony Allee, Pharmacist. The Board approved a Settlement Agreement and Final Order sanctioning Respondent with a citation and warning and a civil penalty in the amount of $250.00. This order resolves the Statement of Charges on June 30, 2016.
Bellevue Pharmacy #4355, Nonresident Pharmacy. The Board filed a Notice of Hearing and Statement of Charges, charging the pharmacy with failure to give proper notice of closure to the Board and failure to give proper notice of closure to patients. A hearing is scheduled for November 1, 2016.

Stephanie Carberry, Certified Technician. The Board approved a Settlement Agreement and Final Order sanctioning Respondent with a citation and warning and a civil penalty in the amount of $100.00. This order resolves the Statement of Charges on June 30, 2016.
Inie Clement, Pharmacist. The Board filed a Findings of Fact, Conclusions of Law, Decision, and Order to dismiss the Statement of Charges filed on June 23, 2015.
Amber Cozad, Certified Technician. The Board approved a Settlement Agreement and Final Order sanctioning Respondent with a citation and warning and a civil penalty of $100.00. This order resolves the Statement of Charges filed on June 30, 2016.
Courtney House, Certified Technician. The Board filed a Findings of Fact, Conclusions of Law, Decision, and Order to revoke her registration. This order resolves the Statement of Charges filed on March 8, 2016.
Melanie Howard, ARNP. The Board approved a Settlement Agreement and Final Order sanctioning Respondent with a civil penalty in the amount of $1000.00. This order resolves the Show Cause Order filed on May 4, 2016.
Craig Johnson, Certified Technician. The Board approved a Settlement Agreement and Final Order sanctioning Respondent with a citation and warning and a civil penalty in the amount of $100.00. This order resolves the Statement of Charges filed on June 30, 2016.

Wendell Simmonds, Pharmacist. The Board filed an Order Granting State’s Motion to Dismiss the Statement of Charges filed on May 4, 2016.
