More High Risk, Already-Inspected Compounding Pharmacies that PACB Accredited But the FDA Found Problems

Prior post have shown that some of the pharmacies that the FDA has inspected as they are high risk compounding pharmacies were given a clean bill of health from PACB. Nonetheless, the FDa during its inspection found violatons. Oakdell Pharmacy was also on the FDA's list of high risk pharmacies and was recently inspected and this report issued. Avella of Deer Valley, Inc, Phoenix, AZ, see here for FDA inspection report  and here for statement that it is PCAB accredited. 

AnazaoHealth Corporation, Tampa Fl, see here for FDA inspection report and here for statement that they are PCAB accrediated.  There statement in fact reads "AnazaoHealth Corporation, Pain Management Division -Tampa, is honored to be among the first in the nation and second in Florida to earn the PCAB Accreditation, as it sets us apart from other compounding pharmacie
 

Again, how did this Pharmacy receive a clean bill of health from PACB but not from the FDA? Maybe a reader familiar with the PCAB accrediaton process can explain the difference in standards and how this is possible.
