A Statement of Charges was issued in January 2013 for failure to comply with its home state rules and laws, disciplinary action in other jurisdictions, failure to comply with Iowa compounding requirements and engaging in the unlawful manufacture of unapproved drug products. This pharmacy was found to have disciplinary action taken or pending in five other states. This pharmacy was found to be conducting three pharmacy operations under one license: Med Quest, Cenegenics, and PhysioAge. The pharmacy was found to have several operational deficiencies as well as dispensing commercially-available products into Iowa. Med Quest was alleged to be manufacturing and distributing unapproved new drug products without FDA approval.
