Some Common Deficiencies Often Found During Pharmacy Inspections

Deficiencies often found during pharmacy inspections include:
1. Pharmacy technicians not supervised by pharmacist;
2. Pharmacy technicians not properly identified with name tags and identified as techs 
3. Medication on shelves not properly labeled (beyond year dating)
4. Sterile conditions not present
5. Controlled substances not accurately recorded on appropriate forms; and
6.  Failure  to keep schedule II inventory and dispensing records separate from schedule III-V records.
