Iowa Board of Pharmacy Actions

Daniel Baldi, DO, entered into a Settlement Agreement and Final Order with the Board. The Board ordered his CSA registration be permanently restricted from prescribing, administering, and dispensing controlled substances for the treatment of chronic pain. In addition, his CSA registration will be restricted for the duration of his probation with the Iowa Board of Medicine.
Wells Pharmacy, entered into a Combined Amended Statement of Charges, Settlement Agreement, and Final Order with the Board. The Board issued a citation and warning for violating the laws and rules governing the practice of pharmacy and ordered a civil penalty in the amount of $12,000.00. In addition, the order requires the pharmacy to remain PCAB-accredited at all times and to verify all prescribers for Iowa patients are properly licensed.
Walgreens Pharmacy 05942, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order with the Board. The Board issued a citation and warning for violating the laws and rules governing pharmacies. The Board furthered ordered a civil penalty in the amount of $1,500.00.
Michael Lyons, Pharmacist, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order with the Board. The Board issued a citation and warning for violating the laws and rules governing the practice of pharmacy. The Board furthered ordered a civil penalty in the amount of $500.00.
Tobin Jacks, MD, entered into a Combined Order to Show Cause, Settlement Agreement, and Final Order with the Board. The Board ordered his CSA registration be restricted during the duration of his Memorandum of Agreement with the DEA.
Mark Pithan, MD, entered into a Combined Order to Show Cause, Settlement Agreement, and Final Order with the Board. The Board ordered his CSA registration be restricted during the duration of his Memorandum of Agreement with the DEA.
McKesson Corporation, Wholesaler 5037, was granted a Controlled Substance Act registration with restrictions mirroring restrictions on their DEA registration.
McKesson Corporation, Wholesaler 5772, was granted a Controlled Substance Act registration with restrictions mirroring restrictions on their DEA registration.
Janice Kirsch, MD, entered into a Combined Order to Show Cause, Settlement Agreement, and Final Order with the Board. The Board ordered her CSA registration be restricted during the duration of her Memorandum of Agreement with the DEA.
