First Question of the Day July 14, 2014 Even as early as 2000, Compounding groups were having to fight the issue of insurance reimbursement for compounded medications and advocated the use of a universal form. Here is a 2000 memo from IACP on the issue.

The memo ends with:  "Further, providers must develop standards that allow for pharmacists
to bill on-line prescribed compounded medications without the use of NDC numbers or reimburse
compounded medications via a written claim form such as IACP’s Universal Claim Form for
