Respondent was also inspected by the Texas Board of Pharmacy, on
December 11, 2012. The Texas inspection report indicates five
operational deficiencies including incomplete drug inventories,
improper drug distribution, improper drug dispensing/labeling,
incomplete policy and procedure manual, and noncompliant sterile
product preparation area. As a result of the deficiencies, Inspector
Meryman issued a Warning Notice to the Respondent on December 11,
2012.
7. Respondent was previously inspected by the Texas Board of Pharmacy,
on September 22, 2011, who observed multiple deficiencies including
improper documentation of pharmacy technician training, improper
distribution of drugs, and inadequate documentation for the
